You will need to complete all application questions. If a question does not apply, you will still need to answer the question with N/A. Once you have completed and submitted your application, you will need to email a photocopy of your driver’s license and a copy of your Student Aid Report. All official transcripts from high school and college/universities you attended will need to be submitted, along with a copy of your acceptance letter to a post-secondary health education program.
